Job Description:
The Phoenix Rehabilitation and Nursing Center is seeking a
dedicated Licensed Practical Nurse (LPN) to join our team of
healthcare professionals. Our center is committed to providing
exceptional care and personalized rehabilitation plans, ensuring
each patient achieves optimal recovery and wellness. As an LPN at
our facility, you will play a critical role in administering
nursing care under the supervision of registered nurses and
physicians, supporting our mission to enhance patient health
through superior care and specialized services. Monitor patients'
health conditions and record vital signs. Administer medications
and injections as prescribed. Assist with patient hygiene, feeding,
and other basic care tasks. Collaborate with RNs and doctors to
provide holistic care. Educate patients and their families on
treatment plans and healthcare needs. Conduct regular assessments
to evaluate patient health changes. Document all patient
interactions and treatments accurately. Strong clinical skills and
knowledge of healthcare protocols. Excellent interpersonal and
communication skills. Detail-oriented with strong observational
skills. Ability to work effectively in a fast-paced environment.
Empathy and patience with a patient-focused mindset. Medical
Insurance Dental Insurance Vision Insurance Valid LPN license from
the state of employment. CPR/BLS certification.
