experience fulfillment at work. Position Overview:The Physician
Assistant (PA) is a key member of the Vanguard Medical Group care
team, providing comprehensive family medicine services across the
lifespan under the supervision of a physician. This position
focuses on preventive care, chronic disease management, acute
visits, and quality-driven outcomes. The PA collaborates closely
with physicians, nurses, and administrative staff to deliver
patient-centered care aligned with Vanguard Medical Groups mission
of improving the health of our communities. Hours: Full-time
Responsibilities: Clinical Care and Patient Management Conduct
comprehensive patient assessments including histories, physical
exams, and diagnostic evaluations. Diagnose, treat, and manage
acute and chronic conditions for pediatric, adult, and geriatric
patients. Order, perform, and interpret diagnostic tests such as
EKGs, laboratory studies, imaging, and screenings. Administer and
document immunizations, injections, wound care, and other minor
procedures. Provide patient counseling and education on treatment
plans, disease prevention, wellness, family planning, and emotional
health. Develop and implement individualized patient care plans
ensuring continuity of care. Use the EMR to document visits
accurately and promptly, meeting regulatory, billing, and
organizational standards. Quality and Performance Accountability
(QIP) Partner with the clinical team to meet Quality Incentive
Program (QIP) benchmarks such as preventive screenings,
immunization rates, and chronic disease management metrics (e.g.,
diabetes, hypertension, cancer screening). Review patient panels
and proactively close care gaps using EMR tools and reports.
Participate in monthly QIP and population health reviews to
evaluate performance and implement improvement strategies.
Contribute to risk-adjustment documentation, ensuring accurate
coding to support quality and financial sustainability. Provide
patient outreach or follow-up for high-risk or noncompliant
patients to improve outcomes. Collaboration and Team Engagement
Participate in TBC huddles, team meetings, and case reviews to
improve coordination of care. Provide input to develop and improve
practice workflows and clinical protocols. Support the training and
onboarding of new clinical team members, including Medical
Assistants and residents. Serve as a resource for clinical staff on
evidence-based practice and regulatory compliance. Compliance and
Professional Standards Adhere to infection-control, HIPAA, OSHA,
and safety policies to protect patients and staff. Maintain
licensure, certifications, and CME credits consistent with NJ state
regulations and organizational policies. Stay current with
evidence-based practices in family medicine and primary care.
Assist in maintaining a clean, safe, and welcoming environment for
patients and staff. Qualifications: Masters Degree from an
accredited Physician Assistant program. Current NJ Physician
Assistant license and NCCPA certification. DEA registration and CDS
license for New Jersey. BLS and ACLS certification (required or
obtained within 3 months of hire). Minimum 2 years experience in
family medicine or primary care preferred. Demonstrated ability to
manage a diverse panel of patients and support quality improvement
initiatives. Strong EMR skills (Athena experience preferred) and
ability to use data to drive outcomes. Excellent interpersonal,

Physical Requirements Must be able to sit, stand, and walk for
extended periods. May require light lifting (up to 20 lbs.) for
supplies or office materials. Must have visual and auditory acuity
to operate computer systems and phones. Compensation and Benefits
The anticipated annual compensation for this position ranges from
$115,000-$140,000, depending on the role and experience level.
Additionally, this position may be eligible for a discretionary
