Pay Information $2,888 per week About The Position Job Title: Registered Nurse - Case Management Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: 8H Days Hours per Shift: 7.5 hours (with a 1-hour lunch) Experience: Minimum of 1 year of inpatient case management experience License: Required Certifications: BLS, PRI Certification, SCREEN Training Certification Must-Have: Experience in discharge planning and inpatient case management Description: The role requires knowledge of discharge planning and making referrals. The position will involve working collaboratively with an interdisciplinary team and patients for discharge planning. Expected to round on the unit with physicians and team members. Candidates must have a positive attitude, strong critical thinking skills, and effective time management abilities. Knowledge of CarePort, MCG, and/or InterQual is preferred. Experience in Utilization Review is a plus. The position primarily involves discharge planning for inpatient care. Common duties include discharging patients to home, community settings, or skilled nursing facilities. This includes obtaining housing, shelter packets for homelessness, hemodialysis packers, and referrals for wound vacs. A resource binder will be provided to aid in these tasks. The caseload typically ranges from 17 to 25 patients and can vary depending on unit configurations. Support from management for breaks and lunch is encouraged. Weekend coverage may be required occasionally with prior approval, and this may involve different caseloads. Holiday coverage may also be required on rare occasions with management approval. Dress code is business casual or any color scrubs, except for specified colors.
