Seeking a Registered Nurse (RN) Field Supervisor to provide case management support, develop care plans, and deliver individualized care to patients in their homes.

Job Requirements

  As a Field Supervisor, you will perform initial and ongoing supervisory field visits to patients' homes, provide assessment, training support, and in-home coordination. You will also convey significant changes in the patient's condition to the clinical care team and document every patient visit in the EMR.
