Job SummaryVNS Health is seeking a skilled Registered Nurse to fill the role of Home Health Field Supervisor. As a key member of our team, you will be responsible for providing clinical leadership and support to our Home Health Aides, ensuring that patients receive high-quality care and exceptional outcomes.Key Responsibilities

• Conduct initial and ongoing supervisory field visits to patients' homes to assess their needs and provide training support.
• Document patient visits and care plans in our electronic medical record system, ensuring timely and accurate charting.
• Orient and supervise Home Health Aides in the field, providing guidance and support as needed.
• Collaborate with our clinical care team to convey significant changes in patient condition, emergency interventions, or care plan changes.Requirements
  • Current license to practice as a Registered Nurse in New York State.
  • Minimum of two years of experience as an RN in a home care setting.
